Register as New Patient

You will need to complete a practice new patient questionnaire, as well as our registration forms. These are available from our patient services team or alternatively you can complete the forms online by clicking on the links below.

Once you have been accepted as a patient, your medical records will be transferred to the practice. Your electronic medical record should be transferred to us within days however, it may take several weeks for any paper medical records to arrive at the practice and be summarised on to your electronic record.

If you are on regular medication, please provide a list of these items  or make an appointment to see our Clinical Pharmacist. This will enable us to stay on top of your health care needs.

The practice has a non-discrimination policy for accepting new patients. There are a number of reasons why you may not be able to register with your chosen GP. For example, you may live too far away. If this is the case, simply choose another GP in your local area.

If English is not your first language, the practice has access to interpreters. Please inform the patient services team at reception and this assistance can be provided.

Temporary Patient Registrations

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the surgery. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
