Self Referral

Most referrals to other specialists will require you seeing or speaking with a GP. However, there are a number of local services, listed below, that you are able to make self referrals to.

Specialist Continence Service

The Specialist Continence Service provides information, support and treatment for adults with bladder or bowel problems. It also provides training, education and advice to carers and health professionals.

This referral pathway won't require a HbA1c reading

NDPP (National Diabetes Prevention Programme)

Patients are now able to self refer without GP referral. To do so patients must access the website. Patients will however need to (a) be eligible (b) have the HbA1c reading from the GP practice.

Crisis line (North Yorkshire). The service is available 24 hours a day, seven days a week for anyone, any age, living in North Yorkshire. Callers are diverted to local crisis teams by following a series of options.

The community directory has information for adults, children, families, young people, parents, carers and professionals working with them. In general, organisations listed in North Yorkshire connect are non-profit community service groups, government agencies, or clubs and organisations. We will not include information about commercial organisations or individuals.

Musculoskeletal Practitioner Service

The MSK Practitioner service is a new service, where Specialist Musculoskeletal Physiotherapy is offered as an alternative to seeing your GP for a back, neck, muscle or trapped nerve pain. The specialist physiotherapist can assess, diagnose and offer advice and guidance on how to manage your symptoms. Our MSK Practitioners are experienced specialist physiotherapist who also work at Harrogate District Hospital. They can also refer you to the hospital for further investigations.

0-5 Children’s Service (Health Visiting)- North Yorkshire

The role of the Growing Health North Yorkshire Health Visiting Service in North Yorkshire is to protect and promote the health and wellbeing of children under five and their families.

IAPT North Yorkshire

To make sure that the IAPT service is right for you, you will need to complete a self-referral form. You can self-refer or via your GP. Assessments are undertaken over the phone, although alternative arrangements can be made if required. At the end of the assessment, we will discuss what help is available within the service.

With the pandemic inevitably impacting on the amount of routine and elective procedures hospitals could perform and patients now coming forward, the number of patients on the waiting lists has increased to around six million nationally. Young Carers

The Young Carers team at Carers’ Resource exists to support young carers in the Bradford, Harrogate, Selby and Craven districts of Yorkshire. A young carer is someone aged 18 or under who helps look after a relative who has a condition, such as a disability, illness, mental health condition, or who misuse drugs or alcohol. Most young carers look after one of their parents or care for a brother or sister.

North Yorkshire Breast Screening Service

The Trust Breast Imaging Unit provides the base site for the North Yorkshire Breast Screening Service which covers the region, It is also the centre for the provision of a comprehensive symptomatic breast service to the population of York, providing one stop and follow up services, as well as some outreach clinics at Scarborough Hospital. All women aged 50 up to their 71st birthday are invited for breast screening every 3 years.
