Primary care networks (PCNs) build on the core work of current primary care services and enable greater provision of proactive, personalised, coordinated and more integrated health and social care for our communities. They are crucial to the development of integrated care systems (ICSs) and in meeting the ambitions in the NHS Long Term Plan.
Mowbray Square Primary Care Network
The Spa Surgery is proud to be one of the 3 surgeries that form Mowbray Square Primary Care Network. We work with our colleagues at East Parade and Park Parade Surgeries to provide quailty health care for the 30,000 patients registered with us.
What is a Primary Care Network (PCN)?
Primary care networks are groups of general practices working together to focus and develop patient care for people living in their area.
How do local PCNs support our patients?
PCNs build on existing primary care services and enable greater provision of proactive, personalised, coordinated and more integrated health and social care for people close to home. Clinicians describe this as a change from reactively providing appointments to proactively caring for the people and communities they serve.
We welcome feedback from our patients and we encourage you to join our Patient Participation Group where you can become an active part of guiding the development of our patient services.
