Registering in Person

Come along to the surgery and ask our reception team for a registration pack.

    • Fully complete the registration form
    • Fully complete the New Patient Health Questionnaire
    • bring photographic proof of identification with you
    • Hand in the completed documentation and identification to our reception team

All new patients are asked to complete a new patient health questionnaire.  If you would like to print and complete this now please click on the link below. 

New Patient Questionnaire

The completed form will need to be brought to the surgery along with the registration form and photographic proof of identity before the registration process can be completed.

New patients may wish to make an appointment with the practice Health Care Assistant for health screening to review medical needs. This appointment and the information provided on your health information questionnaire is particularly helpful whilst we await your medical records. If you have not seen a doctor for some time, it will also help us to assess your current state of health. Please note that we cannot issue repeat prescriptions for new patients until you have had an appointment with a doctor.

Temporary Patient Registration

If you wish to be seen as a temporary patient, please come along to the surgery and ask the reception team for a temporary resident form.  This applies to patients who are away from home or on holiday in the area or if you are not registered with a doctor but need to be seen, you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
